As mentioned in Hendricks County Circuit Court Order Book 122 (February 1976 - March 1977), in October 1976, Lisa Murray Manchester petitioned the court (case # C 76-268) to change her name to Lisa Murray Manchester Phillips. This petition (which was filed on her behalf by Jacqueline H. Phillips) was granted.
